99214 CPT Code: A Complete Guide in 2026

The image portrays the concept of a guide explaining 99214 cpt code

The 99214 CPT code reports an established patient office or outpatient visit with moderate medical decision making (MDM) or at least 30 minutes of total practitioner time on the date of service. For many primary care and specialty practices, it is one of the most frequently billed codes.

Because the line between 99213 and 99214 is often misunderstood, practices commonly undercode chronic care visits or, less often, overcode simple ones. This guide explains the exact 99214 requirements, how to document them, and how to bill the code correctly.

Two ways to support a 99214 visit

Moderate MDM

Meet 2 of 3 elements: moderate problems, moderate data, and moderate risk.

Time

At least 30 minutes of total practitioner time on the date of the visit.

Common fit

Chronic disease follow-ups with prescription drug management.

What Is the 99214 CPT Code?

Since 2021, office visit levels are based on MDM or total time. History and exam should be medically appropriate, but they no longer determine the level. Therefore, a strong 99214 note centers on the decisions made or the minutes spent.

What Are the Moderate MDM Requirements for the 99214 CPT Code?

To reach moderate MDM, the visit must meet two of the three elements below.

ElementWhat meets the moderate level
Problems addressedOne or more chronic illnesses with exacerbation, progression, or side effects; two or more stable chronic illnesses; an undiagnosed new problem with uncertain prognosis; an acute illness with systemic symptoms; or an acute complicated injury
Data reviewed and analyzedOne of three categories: any three of external notes, unique test results, unique tests ordered, or independent historian; or independent interpretation of a test; or discussion of management with an external physician or QHP
Risk of managementModerate risk, such as prescription drug management, a decision about minor surgery with risk factors, or care limited by social determinants of health
Moderate medical decision making elements

In practice, a visit managing two stable chronic conditions with prescription drug management meets two elements and supports 99214. In contrast, one stable chronic illness with a refill is usually 99213. For that level, see our 99213 guide.

Time-Based Coding for the 99214 CPT Code

Alternatively, select 99214 when total practitioner time on the date of the encounter is at least 30 minutes. Time includes reviewing records, history, exam, counseling, ordering tests, documenting, and care coordination. However, it excludes clinical staff time and separately billed services.

For example: “Total time on the date of service: 33 minutes, including lab review, counseling on medication options, dose adjustment, and documentation.”

99214 CPT Code Documentation Example

“67-year-old with type 2 diabetes, hypertension, and hyperlipidemia. A1C rose to 8.2 from 7.1, and BP is 156/92. Reviewed recent labs and an outside ophthalmology note. Increased metformin, adjusted lisinopril, and counseled on diet. Recheck in six weeks.”

This note supports 99214 because it shows chronic illness with progression plus prescription drug management. Moreover, it records the data reviewed, which strengthens the case in an audit.

99214 Billing Guidelines

  • Link every diagnosis actually addressed, not only the primary one
  • Use modifier 25 only for a significant, separately identifiable visit on the same day as a procedure
  • For Medicare, consider add-on HCPCS G2211 when you provide ongoing, longitudinal care
  • Use the correct place of service, such as POS 11 for the office

For more on same-day procedures, review our modifier 25 rules. Payment varies by payer and location, so check Medicare amounts in the CMS Physician Fee Schedule Look-Up Tool.

Common 99214 CPT Code Mistakes

  • First, generic notes such as “doing well, meds refilled” that show no decision making
  • Second, listing only one problem when several were addressed
  • Also, reviewing labs or records without documenting the review
  • Finally, using outdated time ranges instead of the current 30-minute threshold

Regular internal audits help catch these patterns. Similarly, comparing code distribution across providers can reveal both undercoding and overcoding. For new patient visits, see our 99203 new patient guide.

Frequently Asked Questions

What is the difference between 99213 and 99214?

99213 requires low MDM or 20 minutes, while 99214 requires moderate MDM or 30 minutes.

Does prescription drug management qualify for 99214?

It meets the moderate risk element. However, you also need a second element, usually moderate problems, to reach moderate MDM.

How many minutes are required for the 99214 CPT code?

At least 30 minutes of total practitioner time on the date of the encounter.

Can I bill G2211 with 99214?

For Medicare, yes, when the visit is part of ongoing, longitudinal care for a serious or complex condition and the visit is not reported with modifier 25 for a same-day procedure. Notably, CMS allows exceptions for preventive services, vaccines, and annual wellness visits.
