Denied PT CPT codes drain revenue from physical therapy clinics every week. Most of these denials trace back to the same few problems: unclear timed-code documentation, weak medical necessity, and missing or misused modifiers.
This guide covers the codes that are denied most often, why payers reject them, and the documentation fixes that prevent repeat denials. It also explains how to work the denials you already have.
Root causes
Why PT claims get denied
Time and units
Billed units do not match documented timed minutes under the 8-minute rule.
Medical necessity
Notes do not tie each service to measurable functional goals and progress.
Modifiers and edits
Missing GP, KX, or CQ modifiers, or NCCI pairs billed without support.
Fix these three areas and most PT denials disappear.
What Are the Most Commonly Denied PT CPT Codes?
| CPT code | Common denial cause and fix |
|---|---|
| 97110 therapeutic exercise | Units exceed minutes, so record minutes and link exercises to goals |
| 97530 therapeutic activities | It looks like 97110, so describe dynamic functional tasks |
| 97112 neuromuscular re-education | It seems like exercise, so describe balance or proprioceptive work |
| 97140 manual therapy | Necessity is unclear, so name the technique and region |
| 97116 gait training | It is not gait-specific, so document gait deficits and training |
| 97535 self-care training | It overlaps 97530, so show ADL teaching or adaptive equipment |
How Does the 8-Minute Rule Affect Denied PT CPT Codes?
For Medicare and many other payers, billed units for timed codes depend on total timed minutes. Therefore, notes must state the minutes for each timed service and the total treatment time.
| Total timed minutes | Billable units |
|---|---|
| 8–22 | 1 |
| 23–37 | 2 |
| 38–52 | 3 |
| 53–67 | 4 |
However, some commercial payers follow the CPT “substantial portion” method per code instead. As a result, always check the payer’s policy before calculating units.
Code-Specific Fixes for 97110, 97530, and 97112
97110 therapeutic exercise. First, list the exercises, parameters, and the impairment they address, such as strength or range of motion. Then link each one to a functional goal.
97530 therapeutic activities. In contrast, 97530 uses dynamic, functional activities such as lifting, transfers, or reaching tasks. Describe the task and its functional purpose so it does not look like exercise.
97112 neuromuscular re-education. Similarly, document balance, coordination, posture, or proprioceptive training, including the specific deficit and the activities used.
Fixing Denials for 97140, 97116, and 97535
97140 manual therapy. To start, name the technique, such as joint mobilization or soft tissue mobilization, plus the body region and the restriction treated.
97116 gait training. Next, record the gait deficits, assistive device, distance, and cues. Stair training and over-ground walking belong here when they target gait.
97535 self-care training. Finally, show teaching: ADL techniques, adaptive equipment training, or home management. That way, it stays distinct from 97530.
Modifier and NCCI Edits Behind Denied PT CPT Codes
- First, Medicare requires the GP modifier on outpatient PT services
- Next, add KX once the patient passes the annual therapy threshold and care remains medically necessary
- Also, use CQ when a PTA furnishes all or part of the service, as Medicare requires
- Finally, apply modifier 59 or an X modifier only when NCCI-paired services are truly separate, such as different time intervals
CMS updates its therapy services billing rules each year. Consequently, review thresholds and modifier policies every January.
Medical Necessity and Progress Documentation
Above all, payers want to see why skilled therapy is needed. Therefore, every note should connect services to measurable functional goals, record objective measures, and show progress compared with baseline. In addition, the plan of care must be current and certified when the payer requires it.
How to Work Denied PT Claims
First, read the CARC and RARC codes to find the exact reason. Our guide to RARC and CARC codes explains how. Then correct coding or units and resubmit, or appeal with notes, minutes, and the plan of care. Also, watch filing limits; for example, Medicare redeterminations are due within 120 days of the initial determination.
How Dr Biller RCM Reduces PT Denials
Our physical therapy billing services team audits units, modifiers, and NCCI pairs before claims go out. Meanwhile, our denial management team works existing denials and fixes root causes. If you are weighing options, also see our comparison of in-house vs outsourced rehab billing.

Frequently Asked Questions
Which PT CPT codes are denied most often?
In general, timed codes such as 97110, 97530, 97112, and 97140 are frequent targets, mainly for unit, necessity, and modifier problems.
Do I need start and stop times for PT codes?
Medicare requires timed-code minutes and total treatment time, not clock times. Still, some payers prefer clock times, so follow each payer’s rules.
What is the difference between 97110 and 97530?
Basically, 97110 targets one impairment, such as strength. In contrast, 97530 uses dynamic functional activities that combine several movements.
When should a PT clinic use modifier 59?
Use it only when NCCI-paired services are distinct, such as when they are performed in separate time intervals, and documentation supports it.
